Our parish understands that applicants provide important personal information through the screening process. Therefore, the information submitted by applicants is treated with the highest confidentiality. If you are uncomfortable submitting an online Screening Form, please use the PDF option that we provide. Confidential information you submit (such as Social Security number, criminal background information) is only sent to St. Pius X Safe Environment Director (Ginger Farrell) and is not accessible online by anyone at another church, school, entity, or the Diocese. Once a person has been screened and cleared, limited non-confidential information (e.g. an applicant’s name, date of last background check, clearance status, etc.) may be viewed by other parish ministry or program leaders who are responsible for checking a person’s eligibility before allowing their service.To complete a screening form on-line, click here. FAVOR DE IMPRIMIR TRES (3) COPIAS DE FORMA DE REFERENCIA – Es requisito entregar tres (3) Referencias completas para una documentación e investigación apropiada. Recuerde que un familiar NO PUEDE ser una referencia. Le sugerimos que sean compañeros de trabajo, amigos o vecinos; personas que los conozcan bien para que les den buenas recomendaciones para supervisar niños y/o jóvenes.
